form required by Ebbetts Pass Sporting Goods to rent any and all winter equipment. The above person(s) 

is authorized on my behalf to sign all forms and waivers required to rent equipment (skis, sleds, 

snowboards, snowshoes, disc, etc....) for my/our children listed above releasing any and all liability 

while in their care to Ebbetts Pass Sporting Goods, Owners, and Manufacturers. All legal

responsibility signed for on the "Agreement and Release of Liability" falls back to me/us for the day
